Booking Form

Sincere is accepting submissions for five custom pet portraits of which the proceeds will go to helping the animals at the Humane Society of Catawba County. Please fill out all fields completely. This round of booking is not first-come-first-served, so take your time to read all of the information below before you begin the form.

~ This booking form will close on Sunday, November 12th at 12:00 p.m.

~ Arm and leg placement only.

~ Tattoos will only be available in black and gray ink.

~ A link to photos of the pet you would like tattooed is required. Please select clear, high-resolution photos of your pets to increase your odds of being selected. Images need to be available through a link such as Pinterest.

~ The form has a place for a description of your pet’s personality; be as descriptive as you want but please allow for artistic freedom.

~ You will be charged Sincere’s hourly rate of $150 for your tattoo.

~ In the booking form there is space available to discuss size, placement, and your budget. Be as specific as possible.

~ If your entry is approved you will be contacted by Bodhi Joi Studio to schedule. Please note it may take 2-3 business days after this form closes on Sunday for us to reach out to you.

~ All appointments must occur during the months of November and December.

~ A deposit of $100 will be required within 48 hours of Bodhi Joi Studio’s confirmation email to hold your reservation, with no exception. Failure to pay the deposit will result in a forfeiture of your appointment, and your spot will be offered to the next individual in line.

~ If your submission is selected the design will be presented to you the day before your appointment via email. It will not be available before then.
